- Duare Sarkar Registration Number:
This will be given by officials at the camp. Leave it blank if not provided. - Aadhaar Number:
Enter one digit in each box, clearly and correctly.
2. Personal Information / ব্যক্তিগত তথ্য
- Beneficiary Name:
Write your name exactly as it appears on your Madhyamik certificate. - Mobile Number & Email:
Provide an active mobile number (preferably linked with your bank account). Email is optional. - Gender & Date of Birth:
Tick the correct option and write your date of birth in DD/MM/YYYY format. - Family Details:
Fill in your father’s name, mother’s name, and spouse’s name (if married). - Category & Marital Status:
Tick the correct category (General/SC/ST/OBC). If applicable, add your certificate number.
Select your marital status accordingly.

- Address Details:
Enter your full address, including house number, area, post office, ward, police station, district, and PIN code. - Educational Qualification:
Mention your highest qualification with the board, year, and percentage. - Present Occupation:
Write your current status (e.g., unemployed). - Bank Account Details:
Fill in your bank name, branch, account number, and IFSC code carefully.
Make sure your bank account is Aadhaar-linked for payment. - Other Government Assistance:
Tick “No” if you are not receiving any other financial support.
